Cherokee Regional Clinic, on the campus of Cherokee Regional Medical Center, will be offering athletic physicals to area students entering the 8th through 12th grades.
The cost for each athletic physical will be $25 with payment due at the time of the physical. If you believe your insurance covers yearly exams, please verify this with your insurance company and mention it when you schedule your appointment.
Athletic physicals at the Cherokee location will be offered on the following dates: July 26, 28 and Aug. 3, 9, 17.
Please schedule your physicals early as these time slots typically fill quickly. The special rate of $25 per sport physical only includes the physical; it does not include immunization charges should your student require an immunization or booster. This rate will only be offered until Aug. 17. After that date, sport physicals will be billed as a general physical. Athletic physicals at a cost of $25 will also be offered by appointment at the Cherokee Regional Clinic locations in Holstein and Marcus throughout the months of July and August. All students who plan to participate in extracurricular activities must submit a physical and insurance waiver before they will be allowed to practice. These physicals should be considered screening physicals and not complete examinations. Students entering the 7th grade are encouraged to schedule a complete physical prior to competing in Jr. High extracurricular activities. Contact any of the three CRC locations to schedule a comprehensive examination.
Remember to contact your school for the appropriate physical forms, and make sure that the parent/guardian sections are completed prior to the appointment. A parent/guardian is also encouraged to accompany their student to the physicals in case they would require additional tests, immunizations, etc. Students must have a permission slip signed by their parent or guardian prior to receiving a physical.
